QDX - After upgrade to v4.0 in "Secure Mode" the password no longer works.

An issue was reported where the Room Password no longer works after upgrading the QDX from v3.0.1 to v4.0.  The QDX would have been in Secure Mode during the upgrade.  The only recovery option available was to perform a factory restore procedure.
 
Steps to Reproduce:   1)      QDX 6000 with 3.0.1 software, enabled Secure Mode 2)      Enabled "Use Room Password as Remote Access Password" 3)      Set password as "1234", confirmed password "1234" 4)      System updated and reboot 5)      Verified password worked > Once back at the home screen, went into System>Admin Settings> prompted for password and entered, "1234".  Accessed Admin Settings. 6)      Downloaded the "Polycom® QDX 6000™ Software Release v4.0.0 (.exe)" 7)      Ran executable, entered IP address of the system, clicked the "Get Endpoint Information" button, verified the information was correct then hit Upgrade". 8)      Next screen it asks to enter the upgrade key, entered that continued. 9)      Once system was upgraded and back at the PAC, I went into System > Admin Settings > prompted for username and password <Results> ·         Entered "admin" as the username and "1234" as the password> Login Failed ·         Entered "Admin" as the username and "1234" as the password> Login Failed ·         Entered "admin" as the username and <serial number> as the password> Login Failed ·         Entered "Admin" as the username and <serial number> as the password> Login Failed
There were two coding errors found in the v4.0 build which contributed to this issue.  They have been resolved in v4.0.1.
